Because we are a small office and the care we provide requires time, we must collect payment for services at the time of the office visit. As an added benefit for our patients we are happy to send in your insurance claim if you have PPO type insurance (where you may choose your own physician). If you have a health savings account or cafeteria plan we will provide you with a super bill so you can be reimbursed.

Kindly give 24 hours notice if you need to cancel. There is a $35 fee for late cancelations and missed appointments.
